Turns out, this isn't the first time Douglas has used the term. He's called Zeta-Jones his "birthday sister" before, easing fans' concerns. In fact, in 2024, he shared a similar photo with the caption, “To my birthday sister, I love you with all my heart! May your new year be the best! @catherinezetajones ❤️.”

Adding to the confusion, Zeta-Jones' birthday post didn't include a pet name, simply referring to him as Michael. However, a look back at previous years reveals she's used his first name in birthday posts before.

The couple, who met at the Deauville Film Festival in 1998, share more than just a birthday. Douglas was born on September 25, 1944, while Zeta-Jones was born on the same day in 1969.
